a guy on GeorgiaPacking.org got a new RFB and it blew up Sad

I wouldn't call that a kaBoom.  Catastrophic malfunction, yes, but not a kaBoom.
 
I wonder if he followed the gas setting instructions. Still shouldn't mangle the gas system like that regardless, but I'm just wondering.
 
It looked like corrosion or oxidation on the barrel threads where the gas vented, and on the gas piston? What's up with that?

The dealer that I got mine from had 2 RFBs.  One was new and one was used.  I wonder if this guy got the used one and the prior owner used corrosive ammo and did not clean it properly.

Just wondering if anyone else noticed the orientation of the flat side of the piston, wich should be facing in, I'm wondering if it wasn't seated correctly before firing

I agree with liberty - that piston looks corroded AND wet from lube or something - both no-no's.  I wonder why it did'nt crunch (that's not a kaboom, btw) on the first shot?

I'm definitely not saying this happened, because I have no way of knowing, but gun dealers are interested in guns. I can see how a neat new design like the RFB might cross their desk and they might be curious about it.  Take it apart and study it. Reassembly without familiarity with the design would be even more difficult if you didn't want to get greasy fingerprints on the manual when selling a new gun. Most people would try to install the gas piston on the PLR/SU backwards if they were unfamiliar and not paying close attention to the manual.
 
If not the gun dealer, then gun dealers can have teenage sons who are curious about firearms. I can think of all kinds of ways that Kel-Tec could ship a properly functioning RFB and the buyer could receive an RFB that looks right with components installed backwards. That's why I take a new firearm apart, clean it, familiarize myself with it and carefully reassemble it while paying close attention to the manual, and then perform a function test before shooting it for the first time. I don't think most gun owners do that.
 
And I still have no idea why some parts of that RFB look like they have light surface rust and oil on them. Of course, it might be a photographic artifact.
 
It's also possible that there was a bad part that inevitably led to an early failure. It happens, and it's more likely with serial numbers under 500.
 
Just like any other breaking news, the early reports are misleading and usually sensationalized. I'm looking forward to hearing the real story. It's STILL not a kaBoom!

IMO, to ME, MYSELF, AND I, it would appear that the RFB DID WHAT IT WAS DESIGNED TO, AND HELD TOGETHER QUITE WELL GIVEN A CATASTROPHIC FAILURE.
 
I would NOT classify it as a kaboom, and also would have no problems taking that firearm apart and calling KT to get it back to them for diagnosis and evaluation.
 
I am the first critic of a high end firearm giving way, but it did do what it was designed to.  
 
The design appears to be sound.

Looks like he did field strip it days before he took it out. Major bummer. I hope we find out what or who caused it. Especially with new products, much of the time the manufacturers won't fully explain the cause.

K-T had it for 5 days, including the 4th of July weekend, and they replaced the gas block, head, and piston.   Their best analysis is that something was improperly annealed after the heat-treat.  The owner posted this over at Arfcom.

It's likely, seems they outsource the heat treating of those small parts and the people that do it for them aren't too reliable. The warping in the ejection tube got so bad, it forced them to make it out of stainless steel instead. I wonder if they'll need to switch to stainless steel for gas assembly bits?
